ESI Card Details

Hy I am working in a multi national company that have many branches in the India and I have get the ESI card from the company but my brother is in the process to get the card so he want to know about the all documents that are needed when he can issue the ESI card?

The concept of Project Pehchan is to found error free identification process.
Three forms of Identification data will be captured.
I. Descriptive : All details about name, IP number, address, fathers name, etc.
II. Image: Passport size frontal chest level photograph of individuals and his
Dependents.
III. Biometric: Finger prints.

Two sets of cards will be delivered – one for use of the employee and the other for use of their respective dependents.The trigger to initiate the identification process in any case will be IP number.For further details I am uploading a file
