Sir,

I am an Account Holder bearing No.34840100006748, IFSC: BARB0KADAMT, Kadamtala, Howrah Branch. I was intended to be a Member of your Baroda Health. Accordingly, I have filled in proposal form and a cheque of Rs.5906.00 dated 23.2.2017 has been deposited to the said branch. But two months or more, no cheque has been debited in the Account. I am very worried. The Branch Manager, Kadamtala Branch has told me that the same has been sent to National Insurance Co. Limited, Barrackpore on 8.4.2017. But no result has yet been made from their end. Kindly look into the matter so that the issued cheque cannot be expired and ensure me that I can be a member of Baroda Health Scheme.
