Uttar Pradesh Government conducted a state level medical entrance test UPCPMT (Uttar Pradesh Combined Pre-Medical Test).

After qualifying the examination, in different government colleges in the state of Uttar Pradesh qualified candidates take confirmation in medical Courses

Below I am providing you the process to download UPCPMT Form:

You can get the form as and when it was available on website if application submission last date was not over.

Eligibility:

You need to be 17 years of age at the time of taking admission
You need to have cleared Higher Secondary Certificate examination or its equivalent examination with Physics, Chemistry, Biology and English can apply for the examination.
You need to have scored at least 50% total marks in Higher Secondary Examination in PCB (Physics, Chemistry and Biology) subjects taken together to apply for the examination (40% in case of SC/ ST/ OBC category)
You need to have a domicile of Uttar Pradesh and should have completed your basic qualification from recognized schools of Uttar Pradesh only

MBBS – Bachelor of Medicine and Bachelor of Surgery,
BDS – Bachelor of Dental Surgery,
BHMS – Bachelor of Homoeopathic Medicine and Surgery,
BAMS – Bachelor of Ayurvedic Medicine and Surgery,
BUMS – Bachelor of Unani Medicine and Surgery
