To do MS in USA you should have GRE/ TOEFL/ TSE test Scores. Following documents are also required for admission:

Copy of the online application form
Transcripts
Photocopy of the test Scores (GRE, TOEFL, TSE)
Resume.
Financial documents
Letters of Recommendation
Statement of purpose (SOP)
Certificates, Papers etc
