Summa Cum Laude, Magna Cum Laude and Cum Laude (for Medicine)

Star Scholars of BS Human Biology of DLSU-Manila (for Medicine)

Top two (2) recommended students of the Deans of College of Science of DLSU in Manila and DLSU-D (for Medicine) and College of Humanities and Sciences of DLSHSI

Photocopy of High School Report Card (for undergraduate) or Transcript of Records (for Medicine) with General Weighted Average (GWA) / General Point Average (GPA)
